CoNEXTions Outreach Specialist


Location: Cape Girardeau, MO

Pay Range: $13.00 - 15.40/hour

Non-exempt: Full Time - 40 hours/week

Schedule: Monday-Friday 8:00am-4:30pm - Hours may fluctuate dependent upon programming needs.


Minimum Required Qualifications:

  • High School Diploma/GED/HSE required.
  • Preference will be given to those who have at least one year of experience working with at-risk youth and/or the homeless population
  • Must hold or obtain by start date, MO Class E Driver’s License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCC’s Motor Vehicle Record standards policy and have reliable transportation.

The CoNEXTions Street Outreach Program (SOP) and the Outreach Specialist Position:

CoNEXTions SOP is a program who works with, and provides support, opportunities, and services for youth 12-21yrs old who are currently, or are at risk of homelessness, abuse, trafficking, sexual exploitation.

As a Street Outreach Specialist, you would utilize a trauma informed care approach that focuses on safety, trust, choice, collaboration, and empowerment.

For our program to find success, we believe in meeting youth where they are. Street Outreach Specialists go out on the streets of our Cape Girardeau community to areas where youth frequently congregate, seek shelter, etc. and provide youth who are in need with a variety of resources: basic need items, community resource information, as well as just a safe person who is not asking or requiring anything in return for any services/items provided.

For any youth who wants or needs more assistance and support; a Street Outreach Specialists will work along side that youth on a regular basis, providing the necessary support, resources, etc. to help achieve the goals the youth wants to attain.


Job Functions

  1. Maintain positive attitude toward agency and clientele.
  2. Conduct community outreach activities, as assigned, to identify and screen potential program participants.
  3. Assist with service delivery and participant supervision at the SOP program drop-in-center.
  4. Ensure that evidence-based curriculums, as identified in SOP Program Manual, are implemented in providing services to program participants.
  5. Complete assessments and case plans with program participants as outlined by SOP Program Manual, as well as all other required documentation including progress notes, case plan reviews, weekly schedules, referral documentation, etc.
  6. Provide support services to program participants including life skill development, housing assistance, job readiness, education services, and other support services identified as a need for that program participant.
  7. Identify individuals who may benefit from other agency services or other community services and make appropriate referrals to assist individuals in accessing those services.
  8. Identify and implement activities to promote leadership skills with program participants.
  9. Maintain driver’s license and ensure compliance with all laws/guidelines related to vehicle operation to ensure capability of assisting with client transport as needed.
  10. Attend all required internal and/or external training or meetings related to SOP Program.
  11. Responsible for transporting clients in agency and/or personal vehicle as deemed necessary and appropriate to meet program requirements.
  12. Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
