Supportive Housing Worker Team Leader

Location: Cape Girardeau, MO

Supportive Housing Worker (SHW) Team Leader


Location(s): Cape Girardeau, MO - Will also be responsible for assisting with coverage needs for Clustered Apartment Program within other county locations as needed (Ste. Genevieve, Perryville, Marble Hill, and Fredericktown).
Full Time: Non - Exempt
Work Hours: Monday-Friday: 8:00am - 4:30pm. Weekends may be required dependent upon agency and client need.
Salary Range: $17 - 19.40/hour dependent upon relevant specific experiences.


Minimum Requirements:

  • Bachelor's Degree in a Behavioral/Mental Health/Human Services Field OR four (4) years of relevant experience working with a similar population.
  • Must hold or obtain by start date, MO Class E Driver’s License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCC’s Moter Vehicle Record standards policy and have reliable transportation

Clustered Apartments allow for on-site monitoring and on-site interventions and treatment of individuals who require an intensive level of care and support, and who may require skills in the necessities of daily living. Clustered Apartments provide low income subsidized housing assistance to persons with disabilities, mental illnesses and/or poverty.

About the Supportive Housing Worker (SHW) Team Leader Position

Responsible for assisting with the delivery of community support services for individuals receiving services through our Clustered Apartments Program; assist in program enrollment; outreach with program referrals; tracking of program outcomes; monitoring program budget and production; function as a professional staff member of a multi-disciplinary team providing comprehensive assessment and treatment services to clients.


Key Responsibilities and Duties

  1. Assist with coordination of recreational and/or group activities as indicated or available.
  2. Co-facilitate monthly tenant meetings with residents and document outcomes.
  3. Assist with engagement of program referrals and client enrollment.
  4. Assist with orientation training to new hires and ongoing training of Supportive Housing Workers.
  5. Be available after hours or ensure coverage for facility related emergencies.
  6. Provide oversight and direction to Supportive Housing Workers, which includes effective documentation and providing support and feedback as needed.
  7. Assist with tracking and identify improvement needs for program targets/outcomes.
  8. Assist with coverage of service delivery as needed, which may include weekend and evening hours as requested. Service activities could include training on daily living skills, linkage with community resources/natural supports, teaching communication, socialization, coping skills, etc.
  9. Ensure protective oversight of clients, monitor the condition of clients, and follow agency policy and procedure regarding medical emergencies or other emergent events.
  10. Coordinate with supervisor to purchase needed supplies for group activities, office, etc.
  11. Responsible for transporting clients in agency and/or personal vehicle as deemed necessary and appropriate to meet program requirements.
  12. Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.

Minimum Required Qualifications

  • Bachelor degree in social service related field; or equivalent experience working with behavioral health, co-occurring mental health/substance use clients.
  • Must hold or obtain by start date, MO Class E Driver’s License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCC’s Motor Vehicle Record standards policy and have reliable transportation
